Rename and move the subcats out

I think the cat should probably be only about 'Combined birth and death rates' and have the two subcats moved out; we don't usually combine two separate things with "and" in the cat-name and then add the two categories as subcats and this case is especially problematic since e.g. an early death is very different from a birth; it also can't get the combination diagrams cat since it has these subcats etc. I'll rename the cat and move the cats & files accordingly.
