Append[edit]

The proposed text looks fine to me, but there probably should be some explanation like this:

Your derivative must only incorporate works from existing Commons files and your own original work. Incorporation of other copyrighted works is not permitted.

Also, append

Use the [http://tools.wikimedia.de/~luxo/reworkhelper/?lang=en Reworkhelper] to upload your derivative.

Looks good to me. Since some people may come here for uploading a deriv work of a free work not on the Commons (e.g. Flickr), how about a line like "If have created a derivative work of a media file not on the Commons, upload it first and then return to this form."--Padraic 14:55, 26 November 2007 (UTC)
Oh, now begins the purpose-creep... :) Uploading Commons derivs is definitely the most common form of deriv uploading. So let's concentrate on getting that right first. Uploading a Flickr deriv could possibly be part of the fromflickr form. Especially since you need to do the whole "is it under a free license" thing whereas at least with Commons derivs you should be safe. pfctdayelise (说什么?) 15:42, 26 November 2007 (UTC)
