When the acutecondition and fever have subsided, the uterus shouldbe thoroughly curetted, and then irrigated. As to theuse of antiseptics and powerful caustics in the uterinecavity, they are much better omitted. They cannotreach deep into the tissue, anyhow, and their only effectis to cause a local necrosis, and later a slough whichsubsequently provides in itself an ideal culture mediumfor micro-organisms. The tissue removed by curetting in such cases isabundant. The epithelial cells of the glands aremuch swollen, and frequently their nuclei stain poorly. Many are desquamated, and the gland lumen is filledwith cast-off cells and leucocytes. The stroma cells arealso swollen, and separated by an exudate, in whichgreat numbers of leucocytes, chiefly polymorphonu-clear, are seen. At times, typical gonococci may bestained within these leucocytes. The blood- and lymph-spaces are dilated, and some local hemorrhagic areasmay appear. When the curetting has been done, as itshould be, at the end of the acute attack, the picturemay present features less definite than those justdescribed. (Plate XXIIL, Fig. 1.)
