Summary[edit]

Description

This fibrinous exudate containing multiple round structures was seen in a parietal pleural biopsy obtained from an elderly man with a right exudative pleural effusion x 1 month. Clinical work-up (so far) is non-contributory.

The appearance of these structures is consistent with calcium carbonate crystals. They exhibit birefringence with polarization that, unfortunately, is difficult to see in the polarized image. There is an interesting report demonstrating that calcium carbonate particles from the external surface of surgical gloves contaminate epidural catheters. (<a href="https://pubmed.ncbi.nlm.nih.gov/10100439/" rel="noreferrer nofollow">pubmed.ncbi.nlm.nih.gov/10100439/</a>). Inquiry was made as to whether there was a chest tube insertion in this case and the patient did indeed have a chest tube insertion. Therefore, it appears likely that the presence of calcium carbonate in this specimen is the result of contamination of the chest tube from surgical gloves.
