Funchal, 18 de outubro de 1517.
Em nome da Beatíssima Virgem Maria e nela encerrei as relíquias dos Dez Mil Mártires .
D. Duarte, bispo de Duque.
Proveniente do altar manuelino da sé do Funchal e localizado pelo cónego Damasceno de Sousa (Fx. 27 mar. 1922; 03 jul. 2021).

Os cronistas insulares citam o ano de 1516, mas o documento cita 1517, décimo septimo, ano que este bispo igualmente esteve nos Açores.
