Deutsch: Koitusposition im Längsschnitt. Feder und dunkelbraune Tusche, mit Numerierung von Manuskripttexten in schwarzem Blei, im 19. Jahrhundert hinzugefügt. (276x204 mm; K/P 35 re; RL 1907 v) – Über dieser Seite steht die Überschrift: „Ich offenbare den Menschen den Ursprung der zweiten – der ersten, oder vielleicht zweiten Ursache ihrer Existenz“, und am Boden des Blattes: „Hier sind zwei Wesen in der Mitte durchgeschnitten, und was übrigbleibt, wird beschrieben.“ Die Hauptzeichnung illustriert fast ausschließlich traditionelle Ansichten über den Geschlechtsakt; Vorstellungen, die vor allem von Avicenna und Galen abgeleitet wurden. Avicenna hielt unbeirrt an der hippokratischen Ansicht fest, dass der „aktivste und dickste Teil“ des Samens, der in sich das Kreatürliche oder die Seele zum Embryo trägt, aus dem Rückenmark kommt. Leonardos Zeichnung zeigt eine Röhre, die den Samen aus den Lendengegenden des Rückenmarks zum Penis trägt, wo er den oberen zweier Durchgänge besetzt. Der untere befördert Samen aus den Hoden sowie Urin aus der Blase. Das gewellte Aussehen des Uterus rührt von der mittelalterlichen Vorstellung her, seine Höhle sei in sieben Zellen eingeteilt. Die epigastrischen Venen werden gezeigt, wie sie die Oberseite des Uterus mit Blut der zurückgehaltenen Menstruation verlassen und zu den Brüsten tragen, um dort Milch zu bilden.
