Henryk Jan Dominiak - urodził się 22.02.1960 roku w miejscowości Jugów powiat Nowa Ruda. Jako członek zwyczajny reprezentuje Wielkopolski Związek Artystów Rzeźbiarzy - Poznań. Mieszka i tworzy na stałe w Tychach. Od 01.06.1991 prowadzi działalność gospodarczą w zakresie - Artystyczna i literacka działalność twórcza. W roku 2000 złożył egzaminy z przygotowania zawodowego z wynikiem bardzo dobrym i uzyskał prawo używania tytułu specjalisty grafiki reklamowej i wydawniczej. 26.02.2008 roku zgłosił do ewidencji znaku wytwórcy swój znak firmowy i zarejestrował go w Okręgowym Urzędzie Probierczym w Krakowie pod nr ewid. 12 322.
