That is disturbing. In the "how could anyone have that and still be walking around" level of disturbing. I'm guessing either elderly person who didn't realise it was as bad as it was ("And it only hurt a little so I thought it would get better") and/or caretakers who didn't notice (for whatever reason). It's an amazing picture - it would be worth having a paper submitted to the New England Journal of Medicine with that one about "need for constant care of the elderly"
