Confused, backstory perhaps?

Call me pedantic, but how did a couple of lesbians procure semen to swap orally? And what sexual gratification could said lesbians get from trading semen? I appreciate this is hardly a top-level debate, but it confuses me. Maybe the image should be altered, as imo, this mystery distracts from the useful pictoral explanation of a term.
