Deutsch: Wachsende Schädelfraktur: Die Fraktur der Kalotte war primär nur schmal, fissural (linkes Bild, frische Fraktur, Alter 6 Monate). Im Verlauf weitet sich der Frakturspalt deutlich (rechtes Bild, Alter 14 Monate). Pathogenetisch geht man von einer Mitverletzung der Dura mit Verlagerung von Anteilen derselben in den Frakturspalt aus. Dadurch wird die ossäre Überbauung verhindert und durch den positiven Druck von innen der Spalt eben sogar erweitert. Es besteht in der Folge die Gefahr einer Meningozele oder Enzephalozele.
English: Skull fracture that grows with the skull: The original parieto-occipital skull fracture was a narrow fissure (left image: fresh fracture at the age 6 months). During the course skull growth, the fracture gap widens significantly (as shown in right picture at the age of 14 months). It is likely that the dura is also injured, with parts of the dura being pinched within the fracture gap. This phenomenon prevents bone overgrowth into the gap and thus causing gap widening due to the positive pressure inside the gap. As a result, there is a risk of a meningocele or encephalocele (herniation of brain contents outside of the skull). |
