В червлёном поле "донской казак на лихом коне", в золотой оконечности 3 голубые волнистые пояса (реки Дон, Медведица и Хопер).

Утвержден Постановлением Кумылженской районной Думы Волгоградской области второго созыва (#37/192-РД) от 25 января 2005 года.
