This image comes from a text published in 1920 in New York and London and which is now freely available on Project Gutenberg. The license on Project Gutenberg clearly states that it is public domain in the U.S. http://www.gutenberg.org/ebooks/18530 I've been trying to track down some more info on the author of the text it comes from (Eva McLaren Shaw). She was the sister of Dr Elsie Inglis and I think was born in India in 1867. For the text to be out of copyright in the UK in 2015, she would have had to have died by 1945, which I think is quite likely. Her sister died in 1917. I can't find a totally definitive death date for her as yet, but she only published two texts - in 1919 and 1920 and after that there is no mention of her anywhere.

This is the first image I've uploaded to Wikimedia Commons and I was having some problems working out how to use the correct licensing tags. I've updated it now to what I think is appropriate, but any further advice and help would be great.
