В нижнем углу монограмма: DC (Domenico Gustos).

На доске, под изображением, надпись: MITIOR HIC REXIT TRUCE MOSCHICA RURA PARENTE; // DISPO-NENTE SUA, FATO. DE STIRPE SUPREMUS. // M.H.A. (Сей милосердней отца Москвою жестокого правил; Волею властной судьбы он был в своем роде последний).

Над гравюрой, по полю, типографским шрифтом сделана надпись: THEODORUS MAGNUS DUX MOSCOVIAE (Федор Великий Князь Московский) и тем же шрифтом, под гравюрой, стихи: MOSCA LYCAONIAE QUAE NATIO SUBDITUR URSAE, // SUB FUIT, НЕ1С CUIUS CERNIMUS ORA, DUCI. // MITIUS ISTE TAMEN DOMINUM THEODORVS AGEBAT, // IPSIUS AC URSO PAR FERITATE PARENS. // ATTAMEN ET VITAM SIMUL, ET SUA STEMMATA CLAUDENS, // EX AL1IS GENITO SCEPTRA RELIQUIT AUIS (Град обширный Москва, Ликаонской медведицы область, Был под началом вождя, коего видим черты. Кроткую властью своей был Федор отцу не подобен. Тот с медведем самим злостью тягаться бы мог. Жизнь завершая свою, завершил он и род вместе с нею, Скипетр свой передав дедов потомку чужих)1.

Портрет является копией с гравюры работы Джакомо Франко, которую принято считать прижизненной и сходной с посмертным иконописным портретом Федора Иоанновича2. Погрудное изображение царя, в трехчетвертном повороте влево вписано в овал в четырехугольнике. Он представлен в высоком колпаке с пером, в парчовом кафтане с плетеными нашивками на груди.
