High-grade squamous intraepithelial lesion, abbreviated HSIL, also known as high-grade dysplasia. 24-year-old woman with normal conventional Pap in 2003. This one is a ThinPrep. If you're lucky, a case of high-grade dysplasia will be like this one, a conspicuous group of distinct dark cells with variation of nuclear size and shape, heavy chromatin, and high nuclear/cytoplasmic ratio. These can really stand out. If you're unlucky, the cells of HSIL can be infrequent, individual, and bland-appearing. Paradoxically, it's easier to overlook a high-grade lesion in ThinPrep than a low-grade one, and the most serious lesions of all--invasive carcinomas--are the easiest of all to miss. See also
