Čeština: Česky psaný dokument – vyplněný (a orazítkovaný) dotazník s údaji o Marii Churavé (1897–1982) (zásadní je chronologický výčet dob a míst věznění od zatčení až do osvobození), nadepsaný: „Evidenční záznam vyhotovený na základě písemných dokladů hlásící se osoby na základě místopřísežné výpovědi“ určený pro „Léčebnou péči o osoby vrátivší se z koncentračních táborů“, datováno 15.X.1945.
English: Czech written document - filled in (and stamped) questionnaire with data about Maria Churavá (1897-1982) (essential is a chronological list of times and places of imprisonment from arrest to liberation), titled: "Evidential record made on the basis of written documents of the reporting person on the basis of sworn testimony" intended for "Medical care for persons returning from concentration camps", dated 15.X.1945.
