Description

Torsade de Pointes

Diagnostic criteria:

This is a classic 12 lead EKG of a patient with Torsade de Pointes.

It shows

the polymorphic nature of the tachycardia, the long QT interval and the initiation of the tachycardia with a late coupled P.V.C. Torsade is a polymorphic tachycaridia that is associated with prolongation of the QT interval (usually greater than 600 ms) and can be acquired or congenital. This arrhythmia was responsible for Quinidine syncope and is an important cause of sudden death in patients treated with type I and type III antiarrhythmic medications. The electrical mechanism may be related to dispersion of repolarization or to triggered activity associated with after depolarizations.

Besides medications, acquired Torsade can be associated with hypokalemia, and a slow heart rate (complete heart block). This patient was a renal patient on dialysis who was admitted to hospital for an E.N.T. operation on one of her sinuses. The arrhythmia was noted during the operation. At the time of the procedure she was taking sotalol and this combined with her renal failure was the presumed cause of the arrhythmia.

Sotalol has both class II (beta-blocking) and class III (prolonging of action potential) effects. Sotalol is excreted mainly by the kidneys and the dosing intervals should be lengthened when the creatinine clearance is less than 60 ml/min. As with the anti-arrhythmic effect of sotalol, Torsade de pointes is dose dependent and occurs in 0.5 percent of patients who taking 160 mg/day, 1.6 % taking 320 mg/day, and up to 5.8% taking more than 320 mg/day.

The arrhythmia in this patient did not recurred post-operatively with the sotalol removed from her medications.

Description

This is an electrocardiogram recorded from a patient in his 80's after implantation of a pacemaker. This recording was made as part of a routine pacemakers follow-up The pacing system was working well . This is an interesting electrocardiogram which illustrates an important point regarding pacemaker lead implantation. It is clear that this patient's pacemaker is sensing his P waves and following with pacing of his ventricle. What is curious about this recording, is the right bundle branch block configuration of the QRS and the extreme rightward axis seen in lead one. It is for this reason, a routine electrocardiogram should be done after every pacemaker implantation that involves implanting a ventricular lead. In actual fact this patient's ventricular lead was pacing the left ventricle. This lead was probably in a branch of the coronary vein . The path of the lead can be traced by the CT video which was taken after this electrocardiogram was recorded. The patient had a pacemaker system that was working well, in spite of the fact that the pacemaker was pacing the left ventricle. In fact, new pacing techniques are pacing the left ventricle as well as the right ventricle to maintain cardiac synchronization. In this case though , the ventricular lead was removed, without complication, because of an associated infection of the pacemaker pocket.

A new pacemaker was implanted from the right side with the new lead placed in the right of ventricle. This resulted in a markedly different electrocardiogram.

An example of accelerated idioventricular rhythm in a patient who was treated with primary PCI after anterior myocardial infarction due to a proximal LAD lesion. The first 5 beats and last 9 beats are AIVR. In between two narrow beats are seen of which the second beat is probably a normal sinus beat. AV Dissociation can be seen in leads V1 and V2.

Type I morphology is seen in V1, V1 IC3 and V2 IC3. Furthermore, there is a horizontal QRS axis, broad P waves, wide S waves in the lateral and inferior leads and fractionation of the QRS complex in III and aVL.

Description There is only (!) a Type I morphology in V1, so officially this would not meet the criteria for a Brugada Syndrome ECG. Leads should have placed one intercostal space above V1 and V2 to reach the diagnosis (which was later done). Notwithstanding that the Type I morphology in V1 is overwhelming and is accompanied by frequently seen wide S waves in the lateral and inferior leads.

Description There is no Type I morphology to be seen on this ECG. There is however a suggestion of a Brugada syndrome ECG by the RSR in V1 and V2 (could also represent Right Bundle Branch Block) with, typically for Brugada syndrome associated ECGs, ST elevation; best seen in V2. RSR with ST elevation and a positive T wave is known as a ' saddle back' ST morphology. This patient received sodium channel blocking drugs to acquire a Type I morphology and the diagnosis. Note that V5 and V6 are placed one intercostal space above V1 and V2 (V1 IC3 and V2 IC3).
