When there is no displacement or external Avound the diagnosis maybe very difficult, because the symptoms are not distinctive and indicateonly severe injury to the foot, pain, swelling, inability to bear the weightof the body on it, and perhaps crepitus on handling or flexing andextending it. The diagnosis must be made by exclusion of otherinjuries, by localized pain, and possibly by recognition of a displacedfragment, or of independent mobility of the head of the bone. I haveonce found it by the a;-rays, in combination with fracture of the oscalcis and without displacement, when unable to recognize it clinically. If a fragment is broken from the upper articular surface of the bodyit should be removed. If the neck has been broken and the head dis-placed it should be restored to its place, by an incision if necessary, orremoved if it is found to be completely detached. If, in addition, thebody has been rotated in its socket it must be readjusted, or, that failing,removed. If the body should prove to be crushed or comminuted itstotal excision, with or without the head and neck, will probablv yielda much better functional result than conservative treatment. In compound fractures by direct violence removal of the astragalusis indicated because the functional result is likely to be better than aftereven successful conservative
