Text Appearing Before Image:
hepassage has been obtunded by the injection, every eighthours, of three grains of choral, and ten grains of bromideof potassium to the ounce of water, and by the internalexhibition, at the same intervals, of thirty grains of thebromide, ten drops of tincture of cannabis indica, and fivedrops of tincture of gelsemium, and by sitz-baths of wateras warm as it can be borne. It will also be wise to throwinto the deep urethra, about ten minutes before insertingthe bougie, a drachm of a ten per cent, solution of cocaine, with the instrument delineated in Fig. 7, the bulb beingpastel just beyond the compressor urathrse muscle. In many instances it will be found that the inflammation T KE ATMENT 47 and hyperesthesia are finally reduced to a small and prob-ably granular, patch, which proves rebellious to the bougie, but which usually disappears under the application ofastringent remedies. Of these, I prefer a solution of nitrateof silver, carried to the tender spot by a contrivance which Fig. 7.
Text Appearing After Image:
Syringe and perforated bulbous explorer. is essentially that of Felix Guyon,1 and which, as is shownin Fig. 7, consists of a syringe of the capacity of rather lessthan a drachm, and of an ordinary bulbous explorer per- Jull. Gen. de Ther., 1867, p. 501. 48 ATONIC IMPOTENCE. forated at the apex of the bulb. The syringe having beencharged with the solution, and its nozzle attached to theexplorer, pressure is made upon the piston until a drop ofthe fluid appears at the small opening. Wiping this off, theoiled instrument is then carried down until the bulb definesthe inflamed patch—and it does this with the greatestaccuracy—when it is slightly withdrawn, and a few dropsare deposited in the urethra. The bladder should beevacuated before the application of the instrument, and thepatient should be kept in bed and use demulcent drinks fora few hours subsequently. With these precautions, thepain and desire to urinate will usually not last more than Fig. 8. Bulbous nozzle. thirty minutes, bpracticaltreatis1890gros
