llateral circulation. The lymphatics are arranged intwo sets; the upper lies along Pouparts ligament. This upper set drainsthe anal canal, perineum, lower portion of vagina, male and femaleexternal genitalia, and skin of lower half of abdominal wall and glutealregion (see Fig. 59). They frequently become enlarged on one or bothsides in venereal disease (soft chancre, gonorrhea, or syphilis) or in car-cinoma of the penis, anus, vagina, etc. Their suppuration causes themto become firmly adherent to each other and to the underlying femoralvessels, especially in lean people. The close proximity of these large ves-sels must be borne in mind in their removal. The other or lower set of lymph-glands (crural) lies parallel to thefemoral vessels over Scarpas triangle. They receive lymph from theentire lower extremity. When an abscess or any enlargement of theseglands is found, the cause must be looked for along the entire limb to thetoes. Bothof these sets of glands empty their lymph into the deep iliac glands lyingbeneath the iliac fascia. After suppuration and incision of the super-ficial glands, a continuation of fever and septic symptoms should lead usto suspect an extension to the deeper iliac glands. In malignant tumors of the testis the involvement of the inguinal andiliac glands may be greater than the primary growth. The internal saphenous vein (see Figs. 56 and 141) penetrates thefascia lata 3 to 4 cm. (i; to 1J inches) below Pouparts ligament, passingacross the lower edge of the saphenous opening to empty into the femoralvein. This vein is frequently ligated in this region for the cure of vari-cosities of its own and tributary trunks (see Fig. 141).
