The testicle in its descent from theabdomen into the scrotum is accompanied by a pouch of peritoneum, which aboutthe period of birth becomes shut off from the general peritoneal cavity by aclosure of that portion of the pouch which extends from the internal abdominalring to near the upper part of the testicle ; the lower portion of the pouch re-maining persistent as the, tunica vaginahs. It would appear that this closurecommences at two points, viz. at the internal abdominal ring, and at the top ofthe epididymis, and gradually extends until, in the normal condition, the wholeof the intervening portion is converted into a fibrous cord. From failure in the OBLIQUE INGUINAL HEENIA 1125 completion of this process, variations in the relation of the hernial protrusion tothe testicle and tunica vaginalis are produced, which constitute distinct varieties Fig. 694.—Varieties of oblique inguinal hernia.

These are congenital, infantile, encysted, and hernia of the funicularprocess. 1126 SUEGICAL ANATOMY OF INGUINAL HERNIA Congenital hernia (fig. 694, b).—Where the pouch of peritoneum whichaccompanies the cord and testis in its descent remains patent throughout, and isunclosed at any point, the cavity of the tunica vaginalis communicates directlywith tlie peritoneum. The intestine descends along this pouch into the cavity ofthe tunica vaginalis, which constitutes the sac of the hernia, and the gut lies incontact with the testicle.
