In the thigh the small sciatic nerve is reached by the same incision asserves to expose the great sciatic nerve (see Fig. 129). The nerve lies underneaththe fascia but much more superficial than the great sciatic nerve. 45. G-reat Sciatic Nerve. From its position and size the great sciatic ranks nextto the trigeminal nerve in surgical importance. It is offen the seat of neuralgia,which may be either traumatic in origin or more often due to pelvic and spinaldisease. In the buttock the trunk of the great sciatic nerve is exposed in the sanie way asthe sciatic artery. Even in the rare cases in which it divides high up, the external andinternal popliteal nerves lie close to one another as far as the lower third of the thigh.The nerve is found midway between the tuber ischii and the great trochanter coveredby the gluteus niaximus, and lying on the obturator internus and gemelli above, andon the quadratus femoris lower down. Incision corresponding to the middle two-thnds of a line from the postero-inferiorspine of the ilium to the root of the great trochanter, parallel to the incision forligature of the sciatic artery.
