Text Appearing Before Image:
tilaginous consistency, and the glistening, bluish color are notsufficiently characteristic. The friability and tendency to bleedwhen grasped by a tenaculum or when the finger-nail is gougedinto it, are regarded by many of large clinical experience ascharacteristic of cancer, and altogether reliable in making adiagnosis. While much reliance can be placed on these signs, the microscopicexamination of an excised piece of the suspected portion must beregarded as the conclusive test, without which a positive diagnosis 350 SPECIAL DIAGNOSIS is often impossible. When ulceration follows the diagnosis is madewith greater ease. The hard, glistening, irregularly elevatedborder, together with the friability and tendency to bleed whenhandled, leaves little doubt as to the carcinomatous nature. Thereis then little need for the microscope to confirm the diagnosis. A cauliflower groAvth is more readily recognized as malignantthan the infiUrating form, yet papillary erosion must be excluded, Fig. 145
Text Appearing After Image:
Primary carcinoma of the cervix, associated with a large submucous fibroid. and to make a careful differentiation the microscope will often befound indispensable. The greater the clinical experience of theexaminer the larger will be the percentage of cases in which thediagnosis can be made from the clinical signs and symptoms. But there will remain a certain number in which the diagnosis canonly be made by a microscopic examination of an excised piece ofthe suspected portion. (See Microscopic Diagnosis, page 353.) PLATE XLVIII.
