Description
English: About half of US adults have one or more chronic health conditions such as heart disease, cancer, diabetes, and arthritis. If you have a chronic condition, self-management education (SME) can help you develop strategies and confidence to manage your condition day to day. There are a variety of SME programs available.

This 60 second video describes the benefits of SME for people with chronic conditions.
