Well then just reupload the image, place {{Otrs pending}} just below the license and send an email to permissions-commons@wikimedia.org and specify the exact link(it's not my email address, it's used by the whole commons project to receive permission). Edit: I found the following template to display an infobox: w:Template:Infobox_Film

Have you really send the confirmation mail to OTRS? Please look at COM:EMAIL again, write an email using the template given there and try to prove that you are the copyright holder. If you look at the OTRS page (the link is above) you will see it states "An OTRS volunteer will either confirm the permission is OK (in which case you can use a template below)".
